Job description

Our client, Wandsworth Council is looking for an Intensive Outreach Worker to join their team.

You will be responsible for providing advice, guidance and support to vulnerable care leavers aged 18 to 25 with a significant identified housing need. This role will include leading; coordinating and implementing a clear plan of support to ensure care leavers with significant identified housing needs have their needs met.

The job purpose is to work closely with statutory and non-statutory partners to promote positive working, and develop pathways that ensure positive housing outcomes and support for vulnerable care leavers. This will include setting up / sustaining tenancies to prevent repeat homelessness. Assessing and identify all support needs and overcome barriers to ensure access to relevant services. The ability to build positive relationships with both young people and partner agencies is paramount.

You will need to be:

HCPC registered
