Our client, Solihull Council is looking for an assistant team manager to join their Hospital Discharge team.

Situated in the hospital discharge experience of acute hospital and community social work required
Data monitoring
Working across the hospital and community acute and discharge to assess
supervising social worker

Combination of WFH and on site so if it is acute will be on site dependant on day to day requirements

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
