Remedy Recruitment Group are currently seeking an Adults Reablement Team Manager to work within Westminster Council.

Job Purpose

• To manage the delivery of a short term Reablement service to individuals with the aim of promoting and sustaining their independence in the community by directly managing a team of Support Workers. The objectives are to support people, who need help because of age, disability or personal circumstances, to enable them to remain in their own homes with as much independence as possible.
• To ensure independent Living Assessors, Occupational Therapist Care Managers and Social workers complete the Goal Planning assessment process and monitor and review the delivery of the Reablement goals set for an individual both during and at the end of the reablement term.
• To ensure that all activities undertaken by staff - Reablement individual, carer or other people involved follow the Policies and Procedures laid down nationally and locally.

You will need to be:

HCPC registered
