Remedy Recruitment Group are currently seeking a Transitions Social Worker to work within a Local Authority based in West London.

Job Purpose

In addition to a sound working knowledge of assessment and safeguarding processes, you will also bring to our team previous experience of working within a Transitions service and have a good understanding of working with a 'strength based model' of social work as part of your practice.
In addition good recent knowledge of working with the Mosaic database would be advantageous

The Community Team for People with a Learning Disability is an integrated service, and experience of working within a multi-disciplinary team and of working with both statutory and non-statutory services would also be an advantage.

You will need to be:

HCPC registered
