Remedy Recruitment Group are currently seeking an experienced Transitions Social Worker to work for a Local Authority in East London.

Job Purpose

The Team needs a qualified social worker within the Transition Service of the 0 - 25 Special Educational Needs Disability (SEND) Team. The social worker will work with young people who have a disability aged 14 - 25 to support them to manage the changes they may experience during their transition into adulthood. The social worker must have the following skills: good assessment skills, report writing, verbal and written communication, negotiation skills, observation, advocacy, case management.

The social workers must have the following experience:

• Undertaking and compiling Care Act assessments, Care Act reviews and carers' assessments, mental capacity assessments, deprivation of liberty safeguards (Court of Protection) , safeguarding adults, working with vulnerable adults with autism, Learning disability, Down Syndrome, Cerebral Palsy, physical disability, communication difficulties and challenging behaviours.
• The social worker must have knowledge of the Care Act 2014, Mental Capacity Act 2005, Mental Capacity and Deprivation of Liberty Safeguards 2009, Children and Families Act 2014, Special Educational Needs Disability Strategy 2014 and Section 42 enquiry.

You will need to be:

HCPC registered
