Ealing Social Services are looking for a qualified social worker to join their community review learning disability team.

Review Project - CTPLD team is under new management. We are a team of dedicated social workers who are strong in teamwork and offer mutual support and advice to all colleagues. Team management is strong in bringing positive change and promote learning environment. Induction and training will be provided. We are recruiting enthusiastic social workers to support very vulnerable clients with LD diagnosis to review clients' care, review support plans within the strength based model of social care. Successful candidates will be required to complete 3-4 reviews and support plans a week, assist with referrals to CTPLD health team, complete CHC checklists and MC assessments when required. In return we will offer management support and advice. Most of assessments are remotely carried out and home visits take place only if this is the only option and risk assessment will be carried out for every home visit.

• To work as a part of a multi-disciplinary team providing consistent, timely, quality and financially sustainable social work services to adults, their carers and their families.

• To improve outcomes for adults, enabling them to have security, stability and enabling them to develop independence.

• To maintain high standards of professional practice and to provide social work services to adults in accordance with the legislative and regulatory framework and departmental guidance and procedure.
