Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within Croydon Councils LIFE Reablement service.

Job Purpose

• LIFE is a community based team conducting assessments on clients who have come out of hospital. Assessments take place in line with the Care Act and Mental Capacity Act and lead to domiciliary care packages and support for individuals to further their rehabilitation and reablement.
• LIFE is an integrated team with Health colleagues from OT, Physio and Nursing as well as paramedics.

You will need to be:

HCPC registered

Eligible to work in the UK
