Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within Newham Councils Hospital Team.

Job Purpose

Newham Council require 2x Social Workers to work in the Integrated Hospital discharge Hub , the role has 2 functions:

• Facilitating discharges from hospital when patients are medically optimised by arranging short term services
• Assessing customer's when they discharged home for long term packages of care - D2A pathway .

You can be based in the hospital in the Social Work department or you can currently Work from home when completing Face to Face Care Act assessments. ( visiting customer's in their home environment or step down placements ) Office based working is allowed now but adhering to regulations . When covering weekend duty staff will be based at the hospital . You are provided with PPE and have to also follow Govt guidance /regulations around COVID .

• Caseloads - 15 to 20
• Working hours - 36 hour week /7 day service( no weekend enhancement in pay )
• 2 Team managers , 3 Practice Managers , 12 SW practitioners
• Shared Parking Permits for home visits
• Oyster Cards provided for use within the borough using public transport which is very accessible .
• Training and induction will be provided

You will need to be:

• SWE registered
• Eligible to work in the UK
