Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within a Hospital Discharge Team for Sutton Council.

Responsibilities as a Social Worker

* Undertake Mental Capacity Act assessments
* Undertake Care Act Assessments and Safeguarding assessments
* Carry out a care plan
* Undertake a caseload
* Attend meetings
* Contribute to discharge the admission of patients to the Hospital

Requirements of a Social Worker

* Qualified Social Worker BA/DipSW/CQSW or equivalent
* Current registration with Social Work England
* minimum 2 years post qualifying UK experience
