Qualified Social Worker - East Riding of Yorkshire Council - Enhanced Hospital Team - Up to £35ph!

  • The Enhanced Hospital Team covers Hospitals which include , York, Scarborough,Bridlington, Scunthorpe, Goole and those which serve the residents of the within the East Riding of Yorkshire.
  • We are working within the Hospital Discharge Policy which incorperates the Discharge To Assess model of Hospital Assessment.
  • You will be required to complete Care Act complient Assessment to a high standard and produce clear and consise care plans which are strength based and will be shared with system partners.
  • Your hours of work will be between the hours of 830 am and 530pm, this will be on a 7 day work pattern to support the needs of the service.
  • We currently have an office base in Pocklington,and the expectation would be for you to attend the office at least 2x weekly. This will be continuasly reviewed in line with the Covid 19 restrictions. Agile/remote working is promoted within the Team.
  • Assessment completed are either by telephone, Microsoft Teams but our preferance is for these to be completed in person on our Discharge to Assess suite in Bridlington. There is also potential for some visits to Hospitals to complete Mental Capacity and Best Interest Assessment.
