Qualified Social Worker - Solihull MB Council - Discharge to Assess Team - Up to £30ph!

Experienced social worker required to work within a busy Discharge to Assess/Hospital social work team.

Applicants need to have recent experience of working within the new guidance for discharge to assess to ensure that people who have been discharged from an acute setting have a full Care Act assessment, Carers Assessment and long term plans completed within a 4 week timescale.

Applicants need to be able to manage complex cases, complete Mental capacity Assessments as required and safeguarding investigations.

Social worker will be required to complete face to face visits as and when required in line with new visiting guidance and risk assessment.

Preferable if applicants have used Liquid Logic social care system previously.
