Qualified Social Worker - Nottinghamshire County Council - Dishcarge to Assess Team - Up to £28ph!

• We need two Social workers that can hit the ground and pick up allocated work in the Discharge to Assess team South.
• One worker to enable patients to be dischargeed from Hospital and one to work with people that have been discharged with a bridging service to complete a strengths based assessemt to provide services ongoing.
• Assessment required within 72 hours for the ongoing service.
