Qualified Social Worker - North Lincolnshire Council - Adult Social Care Team - Up to £35ph!

Completing Care Act Assessments, Mental Capacity Assessments, best interests chair, leading and completing safeguarding investigations'. Completing comprehensive and detailed case recordings of involvements and decision making. Completing days on the duty system rota and supporting peers and staff in the team. Completing Dol in the community (DOLIC) and other court reports, to a high standard. Participating in the rota to support the front door service and providing specialist support to people on initial referral into the authority and sign post as appropriate. Completing robust care and support plans that focus on self directed support and person centred practice. Addressing housing needs, health needs and liaising with partner agencies to support people.
