Qualified Social Worker - Shropshire Council - Adult Social Services - £28ph - Negotiable Pay Rate!

As a qualified Social Worker you will be responsible for working with older, vulnerable or disabled adults in the central Shropshire area to provide an assessment and care management service which adheres to the Care Act. You will support and advise people with eligible needs to be as involved as possible in how those needs are met by encouraging creative and flexible ways through the use of community resources and personal budgets.

You will support carers by assessing their needs too and providing them with services, advice and support to continue to care for as long as they wish to, and are able. You will give advice and signpost adults with eligible needs to access both specialist and generic services and where appropriate promote positive employment outcomes and achieve settled accommodation by accessing appropriate housing and tenancy support for example.

You will be required to assess and manage risks and review support plans with individuals to set goals, ensure that needs are being met and whether services are still required. You will work as part of a Social Work and team in collaboration with other professionals to provide a comprehensive assessment and care management service in the area.
