Remedy Recruitment Group are currently seeking an experienced Adult Social Worker to work within a Duty Team for a Local Authority based in South London.

Job Purpose

• To have knowledge around CLS and participate in multi-disciplinary working to explore community resources to support residents to draw on resources in their local communities to prevent crisis and enhance their well being.
• To undertake over the phone initial assessments including risk assessments, signposting, respond to enquiries and to implement support packages where required as well as reviewing existing care packages to minimise identified risks.
• Participate in face to face Care Act Asset-Based Assessments in crisis situation and implement any support necessary to minimise the risks identified and ensure practice is underpinned by the principles of the Care Act.
• To triage safeguarding initial concerns, information gathering and participating in SAM discussions to determine whether concerns requires formal Section 42 or transfer for care management.
