Job description
Our client, Manchester City Council is looking for a Qualified Social Worker to join their Adults social Services
A social worker is required to support the HDTA Central team for a 12‑month period covering Intermediate care at Willowbank. The role involves providing direct support to service users and their families in line with statutory duties and national/local policies, ensuring positive outcomes for citizens. Working closely with MLCO partners and other stakeholders, the role holder will assess and manage current and potential risks, and design person‑centred support plans that promote independence.
Key responsibilities include:
- Actively contributing to MDT discussions, particularly around Estimated Discharge Dates (EDD), length of stays etc.
- Supporting patients and families to manage expectations during the discharge and care‑planning process.
- Collaborating with therapy staff to gather assessment information, including for high‑value care packages.
- Keeping the flow lead informed of patient progress and escalating social care concerns through senior practitioners and the team manager as needed.
- Remain on HDTA weekend rota (aligned with unit coverage).
