Job Description:

Our 0-25 Disability service works closely with a range of statutory and voluntary service providers including education, health and housing to ensure a holistic package of support is in place to promote positive outcomes for disabled children and young adults including a clear focus on the young person's transition to adulthood. This may include information, guidance and advice on local, universal provision and services in Camden, Short Breaks (respite), as well as Child in Need, Child Protection, Looked After Children statutory services and Adult personalised budgets for young adults over the age of 18. We currently have 2 Social Work positions, you will work with children, transitions and young adult focused work. The role involves holding a varied and complex caseload of children, young people and/or young adults with severe to profound disabilities. Duties will include working within a multi-agency context to meet the holistic needs of the child, young person and or young adult through comprehensive assessment and review processes, child protection work, child in need work and care act assessments, reviews and planning into adulthood. The role requires a confident approach to working with statutory, private and voluntary agencies and an ability to identify and apply complex systems and frameworks whilst remaining person centred. You will need to be to a person centred and pro-active analytical thinker, with experience of working in an integrated setting with disabled children and young people and experience of safeguarding. You will also need to be a strong team worker, who brings ideas for service delivery and in partnership with parents and young people. You will work in accordance with guidelines set out by local management and to make appropriate use of statutory, independent and voluntary resources. You will demonstrate a high level of professionalism, ensuring professional social work standards are maintained at all times. You will have a minimum of 1 year post qualifying experience and have already completed the Assessed and Supported Year in Employment (ASYE) where appropriate. You will have an excellent working knowledge of the legislative frameworks relating to children and young adults, including recent legislative reforms and how this translates to working practice and positive outcomes for children and young people with disabilities. You will evidence an ability to undertake appropriate assessments, plan interventions, review and evaluate outcomes. You will be able to demonstrate knowledge and understanding of the pressures faced by families with children and/or young adults living in socially deprived neighbourhoods, the issues faced by/needs of disadvantaged families with children and/or young adults. You will have sound experience of undertaking complex assessments of a child and/or young adult's needs including a clear understanding of risk and actions to reduce risk; of designing and implementing care/protection plans and completing court work; of monitoring and reviewing such plans over a period of time. Pay rate can vary between £35-£38 and if more experienced (advanced practitioner that can hold complex cases and has relevant experience)
