Job Description:

The Reablement OT role will involve completing therapy assessments and interventions which are generated and referred by the senior Reablement carers. There is a quick turn over of clients and the OT role is vital in providing next day and urgent assessments and interventions. The caseload is taken between the OT and the OTA (assistant). There is one occupational therapist assistant and one physiotherapist in the team. There are weekly multidisciplinary team meetings every Monday afternoon from 3-5pm. These meetings are currently online due to remote working to discuss the goals and progress of the clients. The Reablement OT referrals are usually for requests to assess the client's activities of daily living and ensuring they have the necessary equipment to meet their goal over a 4- week period. Once the referral is received there is an OT assessment which is available from London Borough of Hillingdon to complete once you have visited the client and set the goals and scheduled a review. The Reablement OT will not hold an individual on their caseload for more than 4-6 weeks, therefore as soon as the referral is received a face to face home visit is required within 1-2 days of receiving the referral to ensure the equipment arrives in good time. We use Medequip on a 5 day delivery service. The Reablement OT role will require a car driver as necessary to drive across the borough. The Reablement OT can also discuss any health needs of the client and refer to the NHS services as we have a good link with the Physiotherapist which is from the NHS
