Our client, North Lincolnshire Council is looking for an Adults Social Worker to join their hospital discharge team.

Working in a fast paced team to facilitate hospital discharges within the new hospital discharge policy (Sept 2020) Discharge to Assess.

Rapid response work completing proportionate assessments in the persons home (or care home) following discharge from hospital.

Identifying care and support needs, utilising the persons support network and arranging services to meet their assessed needs.

Experience and working knowledge/application of relevant legislation is essential for this post including the Care Act 2014, Mental Capacity Act 2005 & NHS Continuing Health Care.

This post will require shift work - we operate a 3 week rolling rota which includes 12 hour shifts and weekend working (1 in 3)

You will need to be:

Social work england registered
Eligible to work in the UK
Atleast 1 year post qualifying experience
