Our client, Cardiff Council is looking for a Social Worker to join their Learning Disability Team.

The case management role involves risk monitoring will include undertaking of a variety of assessments and the implementation and review of care and support plans - as underpinned by the Social Services and Well-being (Wales) Act 2014. The Learning Disability team works with citizens and their families to maximise independence and opportunities which enable service users to enjoy purposeful, meaningful and fulfilled lives. Experience of assessment, care planning, person centred planning and care management are essential, as is the ability to use IT in a competent manner and to meet performance standards. Social Workers work collaboratively with colleagues across a range of agencies and council directorates, from, Education, the voluntary sector/ Providers and Children`s Services. These posts offer a unique opportunity to support adults with a learning disability who may require: Assessments under the Social services and Well- Being (Wales) Act 2014 Care and Support plans Transition planning Court proceedings/ Health and Social Care Legislation Continuing Health Care assessments

You will need to be:

Social work england registered
Eligible to work in the UK
Atleast 1 year post qualifying experience
