Our Client Barnet Council are looking for a Senior Practitioner to join their Learning Disabilities Team.

To provide a high standard of social work to users arising out of the councils duties and powers under legislation and in accordance with divisional and area policy procedures, guidelines and the code of Practice for Social Care Workers.

Working as a member of a multidisciplinary team to provide appropriate, professional social work support for adults with mental health problems within the framework of the social work outcomes.

To act as a care co coordinator in accordance with the mental health trust.

Within the context of an enablement approach and operating within an integrated service, to provide individualised, outcomes-led, personalised support, based on the care plan that inform the setting up and coordinating appropriate packages of support.

Undertake the complex assessments packages or care and long term support for people who are eligible for services that require a professional specialist input.

Essential requirements:

Degree in Social Work or equivalent qualification
Registration with Social Work England
ASYE completed
