Job Description:

CTT Experienced Social Worker Role The Community Treatment Team in Havering is looking for enthusiastic and dynamic experienced Social Workers to come and join this team. The role supports the front door of Barking, Havering and Redbridge University Hospital to prevent, where possible, admissions into hospital. Candidates are expected to have strong hospital discharge experience, and a key understanding of community health and social care services that are available to support people within their own homes. The hospital has a strong emphasis on discharge to assess models, and ensures reablement services are provided to best support a person in their normal place of residence. A key feature of these roles is to provide expert advice and guidance to health colleagues so that a person can be follow up in the community without the need for admission to hospital. This will involve strengths based conversations, involving unpaid carers and advocating for a person who may have issues with their mental capacity. We are looking for people with excellent assessment skills, knowledge of the relevant legislation such as the Care Act, Section 42 enquiries, eligibility criteria and the Mental Capacity Act and an understanding of continuing healthcare process. The hours are 8am to 6pm, and involves weekend work. The shift pattern is often 4 days in a row, with weekend shifts rostered amongst the team.
