Our client, Warrington Council is looking for a social worker to join their Adults team.

An exciting opportunity has arisen for Grade 8 social workers (x2 posts) to apply to Warrington Borough Council Adult Social Care, supporting Adults from 18 years of age to the frail elderly.

Our social workers make a huge difference day in, day out, and our organisation is playing a leading role in efforts to deliver the truly joined up health and social care services the people of Warrington need and deserve.

The post's are (x2) full time posts, working 37 hours a week. The service covers 4 geographical areas in Warrington; South, West, East and Central. Each team is supported by a Team Manager, a Senior Practitioner, Experienced Social Workers, Social Workers (including ASYE) and Community Care Workers. The teams also have access to admin support.

The successful candidate will carry out Discharge to Assess Reviews: bed based Nursing/residential placements and review and identify Care Act 2014 eligibility: Adult social care packages within the community: clients own homes/step down care facilities. The successful candidate will work across Hospital and Community settings, and will work primarily with adults with a range of complex needs including, Mental Health, Physical Disability, Learning Disability and Adults with cognitive impairments: Acquired Brain Injury, Dementia. Workers on the team have the opportunity to develop their skills and knowledge around a wide range of issues including Care Act Assessments, Court Of Protection, Safeguarding enquiries, Continuing Health Care, Best Interest Decisions and Mental Capacity Assessments. You will work within the legislation of the Care Act, Mental Capacity Act and the Mental Health Act in order to deliver personalised assessment, support planning and reviews following the strengths based approach, networking with partners and ensuring holistic outcomes.

Key role: IT experience of Mosaic, experience of completing Care Act assessments, Risk Assessments, MDT meetings, Safeguarding Assessments, Mental Capacity Assessments and Best Interest Assessments. To be able to apply a Strength Based holistic person centred approach, working alongside other disciplines: Health and Social Care Professionals. Complete social care assessments in a timely manner adhering to legislative frameworks. Ensure WBC charging policy is discussed and care packages presented at Panel. Work in partnership with the client and the clients families to promote and maximise independence.

The ideal candidate must be flexible and be able to travel inside and potentially outside of borough.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
