Our Client Waltham Forest Council, are looking for a Senior Social Worker to join their Adults Team.

Jobs Purpose

Social worker for Case management responsibilities. -Care Act assessment and carers assessment of needs. -Developing Outcome focus and cost effective care plans and attending funding panel. -Care review of clients with complex needs, and placement review to ensure care arrangements meeting assessed needs. -Joint working with LD health and completing CHC checklist, attending DST meetings. -Undertaking MCA and best interest process and DOLS application where required. -Undertaking Safeguarding s42 enquiries as the allocated social worker where required. -Coordinating in hospital discharges and attending CTR and high risk meeting if case is allocated.

Essential requirements:

Degree in Social Work or equivalent qualification
Registration with Social Work England
ASYE completed
