Job description

Our client, Southwark Council, is looking for a social worker to join their Older Persons and Physical Disability Service.


The Assessment & Intake Team is based in the community and provides an initial assessment function for the Older Persons and Physical Disability Service. The team works carries out short term pieces of work - up to 6 weeks.

The team complete Care Act Assessments put in place long term packages of care, undertake carers assessment, urgent assessments for care and respite, assess for placement in residential and nursing home settings, jointly undertake some DSTs with nurse assessors, complete mental capacity assessments and undertake the initial concern phase of adults safeguarding before passing any enquiries across to the case management team.

A&I has OTs in the team who support the social work assessments by completing OT assessments, providing advice to the Social workers and providing provision of moving and handling equipment.

Any cases which are complex and have identified and agreed long term social work intervention needs are not held by A&I and would be transferred to the case management team.

About the role:

Main duties will be completing reviews, scheduling reviews, converting care plans to support plans and making sure that the people of Southwark are receiving the support that meets their needs.

Provision of a safeguarding function, to assess and investigate risk, mental capacity, DOLS (LPS) and safeguarding issues ensuring the use of all appropriate resources from Social Care, Health and other voluntary and statutory agencies. Provision of a short term intensive assessment period, assessing the person's ability to maximise their independence and to provide a robust assessment that measures the amount of ongoing support that is needed.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
