Job description
To Lead and manage the integrated Joint Assessment and Discharge (JAD) Service and to ensure effective collaborative working with interdependent teams.
This role will be;
- To provide operational management for the integrated Joint Assessment and Discharge Service
- To provide strategic and professional leadership, including professional development and competencies for respective practitioner
- To facilitate safe return home through collaborative working;
- To provide the integrated health and social care support required to discharge patients with social and/or complex medical needs;
- To identify end of life patients who wish to be looked after at home and ensure that they receive expedited discharge with the right health and social care support;
- To minimise delays arising from problems with inter-agency liaison;
- To focus decision making with the service user at the centre of processes;
- To analyse trends e.g. frequent attenders, borough trends, reduction in bed use, increase in community care packages.
- To be accountable (as the lead organisation) for the delivery of key performance indicators and budgetary control
- To provide a senior operational interface with other senior staff within the health and social care system
- To ensure the consistent application of relevant eligibility criteria Fair Access to Care Services, Continuing Health Care and Rehabilitation
- To be accountable for committing expenditure on packages of care and placements
- To ensure service users, including self-funders, receive the appropriate level of support to plan their own packages of care and are able to exercise choice across a range of options
- To ensure arrangements are in place to provide care and support to people who lack capacity, or where there are safeguarding concerns
- To ensure strategic and operational relationships are productive across a wide range of stakeholders, ensuring JAD aims and objectives are understood by all partners
- To work for continuous improvement of the health and social care systems and pathways
