Remedy are currently seeking a Senior Social Worker (SSW) to work as part of a specialist transition team for a Local Authority based in South London.

This is a 6 month locum social work opportunity to join a specialist Transitions Team that works with young people with special needs, including learning disability, autism and or physical disability who are moving into adult life and from support from Children's services to Adult Social Care.

The post holder will work with young people aged 18-25, undertaking assessments and support planning under the Care Act 2014 and making sure rights enshrined in the Mental Capacity Act 2005 are in place. The overall aim of the post is to support the young people to get a good start in life, looking at their needs holistically, including social inclusion and future goals.

Experience of working with and engaging young adults and people with learning disabilities is required, also the ability to work with families/carers and the range of agencies that may be involved: SEN, Schools, Health, Voluntary sector and other providers.

The work will include Care Act eligibility assessment from age 18 and subsequent reviews and also more in depth work and planning with young people at key points. So this gives the opportunity to do some real relationship based social work and support young people to get a great start in life. The team comprises 4 workers (2 adult and 2 children's Transitions workers and a team manager and works alongside another small review team for people with complex LD and PD needs.
