Remedy Recruitment Group are looking for a Qualified Social Worker for a Transitions Team for Somerset Council.

Job Purpose

Transition social worker required to work alongside transition Adult Social Care Worker to support young people to prepare for adulthood. This role will require an individual with excellent knowledge of learning disabilities. You will reporting to transitions locality lead as well as Community and SEND managers.

You will need to be:

HCPC registered

Eligible to work in the UK
