Qualiifed Social Worker - Sheffield City Council - Preparation for Adulthood Team - Up to £35 + Negotiable!

The Preparation for Adulthood Team (PAT) are looking to recruit advanced social work practitioners to work with young people in Sheffield (aged 14-18+), as they approach adulthood, to support their transition from children's services into adult services.

Workers will be completing social care assessments (in line with The Care Act) and implementing support plans to meet any eligible care needs that a young person may have, as they turn 18, experience of both childrens and adults disabilities services is an advantage.

Social Workers will be required to have knowledge of the Mental Capacity Act and the Childrens Act and how this can impact on key decisions for any young person they may be working with, and will be expected to utilise Universal Services to look at promoting independence for the young people of Sheffield.

Good use of IT skills is essential, services use Liquid Logic.

We are currently working under a hybrid model of part office part working from home and laptop and phone will be provided. Home visits will be required.
