Hillingdon are recruiting for Qualified Social Workers to join their North, South and Central Older Persons and Physical Disabilities Teams. These teams cover Older Adults, including Older Adults with Dementia and other mental health needs and People with Physical Disabilities. Suitable candidates would have experience in Undertaking carer's assessments and developing and reviewing support plans.

Requirements for the post;

• Qualification within Social Care
• Registered with the HCPC
• Completed ASYE if qualified post 2012
• Minimum of 2 years post qualified experience working within in a Older Persons and/or Physical Disabilities Team.
