Job description

Qualified Social Worker - Cardiff Council - Older Peoples Service - Up to £37.37ph!

We are seeking to fill two posts in a new project for Cardiff. In partnership with Health colleagues we are seeking to work to a Discharge, Recover and Assess model when planning hospital discharges from UHW and UHL.

The project will allow you to work with citizens as they move through the discharge pathway, sometimes going home with a care package and at other times placement in a care home. There will be an MDT approach with input from Health colleagues as well as a need to build and develop good relationships with private providers.

This is a very challenging time in social care but we want to see what we can do as an Authority to ensure that citizens are assessed at the right time and with their best interests and their outcomes at the centre of the work we carry out.
