Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within an Adults Mental Health Team for a Local Authority based in West London.

Job Purpose

• To screen and assess new referrals received by other agencies and to recommend appropriate action. This includes requests for Mental Health Act assessments. To undertake initial assessments of new transfers / referrals received by the department and to recommend appropriate action and formulate individual care plans for adults with severe and enduring mental health needs and their carer's. To assess eligibility to services.

• To participate in weekly team meetings and contribute to these. To partake in the duty rota, assessment rota and other tasks as directed by the Lead Practitioner.

• To undertake preventative and supportive work with individuals and families living within the community. To continue to support clients to live as independently as possible in the community within the Recovery Model framework.

You will need to be:

HCPC registered
