Job description

Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker (QSW) to work within a Community Mental Health for an Older Persons team in a Local Authority in North London.

Job Description

  • To carry a caseload consisting predominantly of people with serious mental health problems and complex needs.
  • To undertake comprehensive assessment of need under the NHS and Community Care Act 1990.
  • To work in a multi-disciplinary setting including participating in multi-disciplinary assessments, risk assessments and risk management plans.
  • To be fully involved in all aspects of care planning (including Section 117 aftercare) and to act as Care Coordinator in accordance with the Care Programme Approach. As Care Coordinator post holders are expected to take responsibility for ensuring the delivery of care and all relevant services (e.g. health, social, housing, benefits), providing and monitoring services and reviewing the care plan, its appropriateness and implementation regularly.
  • To participate in a duty service with other members of the Locality Team.
  • To conduct Carers Assessments in accordance with Carers (Recognition and Services) Act 1995 and to ensure service users and their carers take as full a part as possible in case discussions.
  • To liaise and work with other agencies and organisations including Housing, Voluntary Sector, other Health and Social Services teams, attending meetings and case conferences as required.
  • To establish and maintain close working relationships with local GP practices and health centres
  • To input and extract data on the computer (RIO and Carefirst), ensuring all client contacts are up to date and the confidentiality of all electronically stored data is maintained in line with the Data Protection Act.
  • To attend regular team meetings and other meetings as required by the Team Manager
  • To have regular supervision provided by a manager from Health or Social Services.
  • To record unmet need and ensure this information is given to the Locality Manager.
  • To understand and at all times carry out his/her duties with due regard for the Equal Opportunities Policy of the Local Authority and the Health Trust.
  • To undertake any reasonable tasks requested by the Team Leader or Locality Manager.
