We require an experienced Social Worker to work within an Adults Learning Disabilities team based within London Borough Of Havering. The role will be required to undertake a portfolio of activity established from the Joint Commissioning Unit, the post holder will be directly interacting with the lead Commissioner for Disabilities as well as reporting to the Operational lead for Disabilities. To be successful for this role, you are required to have a minimum of 2 years recent Social Work experience of working within an Adults Learning Disabilities team.

The successful Social Worker will have experience of:

• Managing a caseload of people with multiple and complex and profound learning disabilities.
• Working in partnership with other professionals devising individual support & care plans based on needs and risks.
• Working with vulnerable adults with making Best Interests decisions under 'The Mental Capacity Act'.
• Conducting assessments
• Writing reports
• Conducting reviews
• Evidencing value for money
• Negotiations with providers
• Developing outcomes for individuals
