Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within Ealing Councils Hospital Assessment Team.

Job Purpose

Ealing Council are presenting an exciting opportunity, for 3 Social Workers to join the Hospital Assessment team as a locum Social Worker. The team is formed of 1 Manager, 4 Seniors Social Workers and 12 Social Workers currently. The nature of the work we do is fast paced and we require a person who discerning. We are looking for a someone who is diligent, vibrant, and knowledgeable in hospital discharge. The team requires individuals who have experience in assessing care and support, who is confidence, and able to demonstrate good communication skills.

Paid at time and a half on Saturday and Sunday.


Working Days: 21hrs 3 days, Saturday Sunday, and Monday. Weekend Working availability is required.

Location: Currently Working from Home.

Role:
* Social Workers will be to complete section 9 assessments and section 27 reviews, following hospital discharge. On the weekends primarily and Mondays.
* Identifying, vulnerable adults and advocate for ongoing care and support.
* Providing handover to maximising their potential to live independently, following assessment.
* Currently the hospital team are prioritising the discharge to assess model. Experience in this sector is welcomed, but not mandatory.

What is required:
* For your application to be considered you must be able to demonstrate the following:
* you should be a qualified social worker with a current Social Work England registration.
* You'll bring at least 2 year's social work experience, evidence of continuing professional development, and capability of practice in accordance with current professional standards.
* Completed/ exempt from AYSE.
* Have the capacity to work Monday, Saturday, and Sunday
