Job description
Our client is looking for a dynamic social worker with the necessary skills, experience and vision to work within a busy social work team at the Local Authority Hospital. This role is to support discharge planning for Adults using the Discharge To Assess Model with an Integrated Discharge Team.
You must be able to work within a pressurised environment and be flexible with working to ensure the core hours are covered. We are currently looking for a worker to cover Monday - Friday (37 hours per week) covering the two areas of the discharge from hospital system.
You will ideally have experience in working within a local authority setting and be able to demonstrate your commitment to effective team working. As an experienced social worker you will have: - excellent communication skills; be proficient with using various computer systems and programmes; a good manager of time and be able to work under pressure working experience of knowledge of working with adults and older people; experience of safeguarding; and experience of undertaking Mental Capacity Act assessments. This post is to support with discharge planning and admission avoidance in the Hospital, requiring attention to a strengths based approach.
