Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker (QSW) to work within a Hospital Discharge Team for a Local Authority based in West London.

The ideal candidate will have at least 2 years post qualifying experience within Hospital Discharge and the Complex Support Planning Team.

- HCPC registration required

- Valid DBS

- Experience within Adults Social Services
