Qualified Social Worker - Doncaster Metropolitan Borough Council - Hospital Disccharge - Up to £28ph!

  • An exciting opportunity has arisen for qualified Social Workers to provide discharge facilitation services within the Integrated Discharge team based at Doncaster Royal Infirmary or in peoples' own homes. The successful candidates will provide a high quality, integrated, effective social work service to adults and carers in accordance with the Care Act 2014 statutory requirements, Mental Capacity Act 2005, local policy, procedure and guidance.
  • The role involves assessing patients' needs to ensure a safe, timely discharge from the hospital setting. You will work in a busy environment and skills in organisation prioritising work and a flexible approach are essential along with excellent recording skills meeting GDPR requirements.
  • The post holder will need to be able to work autonomously using initiative to manage own complex caseload. The roles will involve undertaking holistic, strengths based assessments with adults, carers and families to help them make informed choices and decisions, enabling them to clarify and express their needs, manage risk and contribute to service planning. You will provide written and verbal records and reports which are concise, factual and informative based on analysis of evidence which can be used within the legal context and where necessary give evidence in court in relation to care or other proceedings. You will use evidence based practice and a strengths based conversation to identify need and take appropriate action which is outcome focused.
  • The role is for 3 months to support discharge from the acute setting to assess in peoples' own homes for longer term needs or similar settings after discharge.
