Qualified Social Worker - Knowsley Borough Council - Hospital Disharge Team - Up to £32ph!

• Social worker to complete continuing health care assessment alongside health colleagues.
• Role will be based within in the hospital discharge team which will include hospital assessment/ discharge, review of 24 hour care placements.
