Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within a Hospital Discharge Team for a Local Authority based in South London.

Job Purpose

1. Experience within an Acute hospital Discharge setting
2. Saturdays working based on a rota
3. Working knowledge of the Care ACT 2012 ( Delayed Discharges, Completing care and support Assessments),Mental capacity Act 2005 and DOL's.
4. Holding a case load
5. An understanding of the Safeguarding Process
6. Attending MDT'S ( Multi-Disciplinary Meetings, Decision Support Tool assessments)
7. Completing reviews of Reablement Packages of care and Placements
