Qualified Social Worker - West Northamptonshire Council - Discharge to Assess Community Team - £32ph!

It will be a mixture of remote working but also visiting provisions

Candidates will be required to conduct face to face assessments to those who have been discharged from a hospital and are either in a residential home or their own home (risk assessment and PPE provided). Office location is One Angel Square, however travel throughout the borough is required.

This was via the discharge to assess process, so they all need assessments to understand eligibility and long-term support needs

Once assessed, the worker would work with the person, their family and the brokerage team to either make the existing placement permanent, move to an alternative provision or return home with a CCP.

This would require the completion of a care act assessment and other supporting pieces of work as required (e.g financial assessment/ mental capacity assessment etc)

Suitable applicants will already be working in a similar field, or have enough experience like for like that they are able to 'hit the ground running'

All candidates must be double-vaccinated, and be prepared to provide evidence to include the dates that the vaccine was received. Without this, candidates will not be considered.
