Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within an Adults with Disabilities Team for a Local Authority based in West London.

Job Purpose

• To contribute to the delivery of an effective service to adults with learning disabilities as part of a team improving outcomes for people with learning disabilities and their carers. To maintain high standards of professional practice and to be responsible for delivering s social work service in accordance with departmental guidance and procedure
• To assess and investigate new referrals for assistance received by the department and provision of services to meet the customer's needs
• To undertake preventative and supportive work with individuals and families living within the community, in partnership with other professionals
• To undertake social work with customers who have been referred to the department
• In the provision of service, develop needs-led packages of care in relation to the resources available. Seeking the most effective means of providing innovative care packages within budgetary constraints in conjunction with partners and external agencies
