Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within a Transitions Team based in South East London.

Job Purpose

Working as a Social Worker on the Transformation Team, which is focusing on efficiency projects within Direct Payment delivery for Integrated Disability service and Older Peoples services.
Key tasks are to undertake Care Act assessments, develop support plans, embed new DP policies and guidance challenge misuse of funding streams. Identify alternative funding streams and increase independence.

You will need to be:

HCPC registered

Eligible to work in the UK
