Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within an Assessment and Care Management Team for a Local Authority based in East London.

Job Purpose

Access to Adult Social Care Team is part of the Assessment & Care Management Service. This team provides a comprehensive telephony and Mailbox front office single point of contact for all adult social care referrals. It is a fast paced busy team Essential Requirements:

1.Good knowledge and understanding of legislation and the national agendas relating to:

  • Ordinary Residence
  • No Recourse to Public Funds
  • Mental Capacity Act
  • Best Interest Decision Making
  • Court of Protection
  • The Access Team are one of two front doors into Adult Social Care, the other being the Hospital Team
  • Referrals are received within the team via telephone, post, fax, email and person - customer, relative, worker, Hospital
  • We deal with new and existing ASC customers presenting with potential needs
  • For existing customers we would check where they are in the pathway, if allocated to another team the enquiry would be passed on to them.
  • A screening contact is carried out to establish if the customer meets the National Eligibility threshold for services to be provided. Customers who do not meet the National Eligibility threshold will be given relevant Information, Advice and Guidance and signposted where applicable to other services
  • Depending on the presenting needs, customers will be transferred to the Enablement team, Supporting Care Team or a assessed under the Care Act
  • Complete 6 weeks Review, present to funding panel approval
  • The team deal with all Safeguarding Concerns and MERLINs Concerns from the Police, they will screen these to decide if a further investigation is needed and transfer to the Safeguarding Team or Safeguarding Governance to deal with

About you

  • HCPC registration
  • Experience of working within Adults Social Services
  • Enhanced DBS
