The roles involves:

• Carrying assessments, support plans and reviews
• Undertaking safeguarding investigations
• Compiling safeguarding reports
• Care Act 2014
• Completing continuing health care assessments
• Mental Capacity Assessments and best interest meetings
• Risk Assessments
• Creating Care Packages
• Facilitating safe and timely discharge of patients into the community
• Completing Section 5 and Section 2 assessments
• Working to the Delayed Discharge Act 2003, Care Act 2014 and NHS Community Care Act 1990
• Carry out mental capacity assessments and best interest decisions
• Attending multi disciplinary meetings
• Compiling reablement plans and primoting independence
