Remedy Social Care division are currently looking for a number of qualified social workers (QSW) to work in the Hospital Discharge team across various different hospitals through local authorities in London.

• You will be required to carry out assessments and creating care plans.
• To ensure safe and effective discharge from hospital setting though undertaking multi-disciplinary assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.
• To apply social work ethical principles and values to guide professional practice and respect diversity.
