Job description

Our client is looking for two experienced and enthusiastic Occupational Therapists to work with their Discharge to Assess Single Point of Access Team. Over the last 12 months, the Adult Social Care department has been developing a 'discharge to assess' offer to help people who are ready to be discharged from hospital, but who still need some support, to get home more quickly by assessing their support needs in the community.

The Single Point of Access (SPOA) are a multi-disciplinary team providing the interface between our hospital partners and adult social care in the project. The OT role will involve completing home visits to assess needs, develop Reablement goals, and implement a rehabilitative care program. It will also involve identifying and implementing risks, prescribing equipment and utilising our Social care system to document actions/assessments. The OT role will also require you to work with your service users throughout their time in Reablement to help achieve their goals.

In addition, the OT will work closely with our hospital partners to assist with discharge, which includes screening referrals via telephone.

There are excellent development opportunities available in this role and as Discharge to Assess team enters its first full year in operation, there is a real opportunity for the post holder to shape, develop and improve the service design.
