Job description

The Community Independence Service (CIS) is an integrated health and social care service designed to provide support to adults with complex health and social care needs. Our aim is to keep people safer at home for as long as possible and avoid them going to hospital if it is not necessary.

The service is made up of a multidisciplinary team of community nurses, social workers, occupational therapists, GPs, geriatricians, mental health workers, reablement officers and others.

We provide urgent nursing and therapy care; supporting people with either disabilities or recovering from injury to help them develop the confidence and skills required to remain independent at home.

To undertake the customers' functional assessment upon referral to the appropriate Team within the Community Independence Service (CIS) using the appropriate assessment & review tools.

To set goals, coordinate support and review customers in Community Independence Service (CIS) to ensure they are as independent and safe as possible undertaking their activities of daily living.

To work alongside members of the wider multi-disciplinary Community Independence Service (CIS) & Adult Social Care Service in a way that is most beneficial to each individual service user.

To work with external partner agencies and individuals to optimise partnership working in the best interests of each service user.

To working flexibly within Community Independence Service (CIS) within the spirit of the services ethos to offer the right service, by the right person, at the right time.
