This role is;
To be able to undertake Family Support Assessments in partnership with families of disabled children.
To take case responsibility and provide practical and supportive services to those service users for whom they are the allocated worker.
To provide families to access service within the Local Offer including support around access to employment and training, welfare benefits advice, housing etc.
To work with parents and/or children in groups around specific issues as identified within the Family Support or Social Work Child in Need Assessment.
To be available on rotation to support the delivery of the Disabled Children's Resource Centre advice, information and support offer.
To write reports, maintain records and in relation to casework. To organise and chair Team around the child meetings for allocated cases. To attend and represent the SEND service at non-complex EHCP meetings.
To liaise, on behalf of children and their families, with, key professionals working in a multi-disciplinary manager in line with EHC Planning.
To have comprehensive knowledge and understanding of the Social Care Local Offer for disabled children in H&F, to support all members of the SEND service.
