The Reablement Service provides successful, person focused short term support to adults within their homes, in order to minimise their care needs and maximise their independence.

We are a fast moving responsive service and are looking for dedicated individuals, with a passion for care provision, who want to make a positive difference for the people living in their local communities.

We are currently recruiting an experienced adult social worker, who will endorse and act in accordance with the principles of personalisation, ensuring that care and support are person-centred and as far as possible put the people with whom they work in control of their lives.

About you

You will be an experienced Social Worker who can promote independence and monitor and evaluate the service delivery.

In this role, you will be expected to take the lead on a mixed caseload, supported by a multi-disciplinary team, to ensure that the care provided directly meets each individual's needs.

You will be confident at undertaking consistent high quality assessments identifying often complex needs, effective planning and implementing the required action in the best interest of the service user.

Your outstanding communication skills will not only benefit your direct work with service users but also your relationships with colleagues and third parties when assessing needs and planning care.

Based in the local community in North Cambridgeshire and Fens, you will be required to travel across the county to support our satellite offices and will need access to a vehicle.
